Health Card

• Do visit the Reception counter No 2 in the OPD Building with your Aadhar card and linked Mobile number for attaining the UHID of FMMCH & Abha number for the Government unified health portal number
• Please write the complete name and address legibly in the form or the link provided at the counter.
• Payment is to be made only through cash or UPI at the MRD Counter.
• Please collect your single Father Muller Registration and Health Card thereafter .
• Any further visits the rules of the Health Card will follow.

• Rs. 200/-for per individual per year.
• No family scheme available.

• The validity of the Health card is for one year from the date of registration.
• Once a duly completed application along with annual fee is received. no request for cancellation / transfer / refund will be entertained.

• The Per visit charges of OPD visit without the Father Muller Health Card is Rs 300/- (Rupees Three Hundred).
• With Father Muller Health Card, the following Registration per visit charges will be applicable
  • OPD visits to General Speciality Departments
    (General Medicine, General Surgery, Ophthalmology, Orthopaedics, Paediatrics, OBG, Dermatology, Psychiatry) 100% concessions will be applicable on registration and subsequent visits.
  • OPD visits to Super speciality Departments
    (Cardiology, Medical/Surgical/Radiotherapy Oncology, Endocrinology, Nephrology, Urology, Respiratory Medicine, Interventional Radiology, Plastic Surgery, Neurology, Neuro surgery, Dental, Spine surgery, Hand & Micro, Speciality clinics) is at 50% concession rate of Rs 150/- (Rupees One Hundred and Fifty Only) per visit.
• Only the card holder is authorised to use the services of the hospital.
• In the event of misuse of the card, the membership to the scheme is liable to be cancelled and no refund will be made.

This Card will be applicable for those getting admitted/treated as Father Muller Hospital patients only.
* Card has to be produced to the OP - MRD (OutPatient Medical Records Department) counter No 3 A-E before availing any service from the hospital.
No card benefits will be given after availing the services.
*Issuing of Father Muller health card and granting facilities which are not mentioned above are at the discretion of the management. In this regard the decision of Management is final and binding.
