• Conduct Certificate from the Head of the Institution last attended*
• Attempt Certificate*
• Registration Certificate*
• University Convocation Certificate*
• Eligibility Certificate from RGUHS*
• A Demand Draft for Rs500/- towards the application fee in favour of Father Muller Homoeopathic Medical College payable at Mangalore
• Five (5) passport size photographs and 3 stamp size photographs
• Migration Certificate
